ONLINE APPOINTMENT REQUEST PROCESS
  1. Select a Date and Time: To request an appointment, simply click the "Request an Appointment" button below. You will be prompted to select a date and time that works best for you. 
     

  2. Credit Card Required: Please note that you are required to have a credit card on file when you schedule your appointment. 
     

  3. Confirmation Email: After selecting your desired appointment slot, you will receive an email from Simple Practice (yourprovider@simplepractice.com), my secure client portal. This email will contain further instructions and links to complete the necessary practice documents. Please be sure to check your email, including your spam folder, for this important message.
     

  4. Complete Required Documents: Please ensure that all required practice documents are completed promptly. As part of the document submission process, you will need to upload a picture ID and provide both the front and back copies of your insurance card. Until all practice documents, including the picture ID and the front and back copies of your health insurance card, are completed, the requested appointment won't be confirmed, and it may become available for others to book. 
     

  5. Appointment Confirmation: Once you've submitted your documents and they have been reviewed, your appointment will be confirmed. At this point, you will receive a confirmation email, and your chosen appointment slot will be reserved for you. 

I kindly request that you complete the forms within 72 hours of receiving them to ensure a smooth process. 

 

This process is designed to ensure that we have all the necessary information to provide you with the best possible care and ensures your continued access to the client portal.
